Walk into a health shop or scroll through wellness pages and you will see "hydrogen water" and "alkaline water" used almost interchangeably. Some ionisers are sold as both. Some bottles are labelled "alkaline hydrogen". It is no surprise that many South African shoppers assume they are the same thing with different names.
They are not. Hydrogen water and alkaline water are based on two different ideas, measured in two different ways, and backed by research of quite different quality. If you are deciding where to spend your money, understanding that difference matters more than any label. This guide explains it plainly.

What is alkaline water?
Alkaline water is water with a higher pH than ordinary drinking water, usually around pH 8 to 9.5. pH measures how acidic or basic a liquid is on a scale from 0 to 14, with 7 being neutral.
Water becomes alkaline in one of two ways:

What is hydrogen water?
Hydrogen water is ordinary water with dissolved molecular hydrogen gas (H₂) added to it. Its pH can be neutral; what matters is how much hydrogen gas is dissolved, measured in parts per million (ppm).
Hydrogen is added through electrolysis in a bottle or generator, or by dropping a magnesium-based tablet into water. Does alkaline water contain hydrogen?
Ionised alkaline water can contain some dissolved hydrogen, but usually not much, and the amount is not the focus of the device. Because ionisers use electrolysis, hydrogen gas is a by-product. However, these machines are designed to raise pH, not to maximise hydrogen, and the hydrogen level can vary a lot depending on the machine, flow rate and source water.
Mineral-based alkaline water, such as bottled alkaline water, generally contains no meaningful hydrogen at all, especially after it has been bottled and stored.
This is the source of most of the confusion. If molecular hydrogen is what you are after, choose a product designed and measured for hydrogen output, rather than assuming an alkaline product delivers it.

Which has stronger scientific support?
Neither should be described as proven, but hydrogen water has the more active research base. Studies on molecular hydrogen have looked at areas such as exercise recovery, metabolic markers and oxidative stress, with some encouraging results and some neutral ones. Sample sizes are often small, so the evidence is best described as promising and developing.

Can you drink both?
Yes. For most healthy adults, neither is harmful in normal amounts, and they do not cancel each other out. That said, very high-pH water consumed in large amounts is not recommended, and people with kidney conditions or on certain medications should check with a healthcare provider before making either a daily habit.
The more useful question is what you are trying to achieve. If you simply want water you enjoy drinking, either may help you drink more. If molecular hydrogen is the specific reason you are interested, focus on hydrogen output.
